1996 5.7L Siverado with 160K miles.

I have recently developed a loud, rattling knock at mid level rpm, around 2500. I haven''t put much time into this one but it''s coming from the back side of the engine. Could it be a bearing? It''s no the typ of know you get when you don''t give it enough gas. It does sound like a deisel engine when it knocks.

Could this be something top side or does it sound more like a deeper issue? Is it time for a rebuild or a new truck?
